ABSTRACT

BACKGROUND: Chikungunya virus (CHIKV) has spread across Brazil with varying incidence rates depending on the affected areas. Due to cocirculation of arboviruses and overlapping disease symptoms, CHIKV infection may be underdiagnosed. To understand the lack of CHIKV epidemics in São José do Rio Preto (SJdRP), São Paulo (SP), Brazil, we evaluated viral circulation by investigating anti-CHIKV IgG seroconversion in a prospective study of asymptomatic individuals and detecting anti-CHIKV IgM in individuals suspected of dengue infection, as well as CHIKV presence in Aedes mosquitoes. The opportunity to assess two different groups (symptomatic and asymptomatic) exposed at the same geographic region aimed to broaden the possibility of identifying the viral circulation, which had been previously considered absent. METHODOLOGY/PRINCIPAL FINDINGS: Based on a prospective population study model and demographic characteristics (sex and age), we analyzed the anti-CHIKV IgG seroconversion rate in 341 subjects by ELISA over four years. The seroprevalence increased from 0.35% in the first year to 2.3% after 3 years of follow-up. Additionally, we investigated 497 samples from a blood panel collected from dengue-suspected individuals during the 2019 dengue outbreak in SJdRP. In total, 4.4% were positive for anti-CHIKV IgM, and 8.6% were positive for IgG. To exclude alphavirus cross-reactivity, we evaluated the presence of anti-Mayaro virus (MAYV) IgG by ELISA, and the positivity rate was 0.3% in the population study and 0.8% in the blood panel samples. In CHIKV and MAYV plaque reduction neutralization tests (PRNTs), the positivity rate for CHIKV-neutralizing antibodies in these ELISA-positive samples was 46.7%, while no MAYV-neutralizing antibodies were detected. Genomic sequencing and phylogenetic analysis revealed CHIKV genotype ECSA in São José do Rio Preto, SP. Finally, mosquitoes collected to complement human surveillance revealed CHIKV positivity of 2.76% of A. aegypti and 9.09% of A. albopictus (although it was far less abundant than A. aegypti) by RT-qPCR. CONCLUSIONS/SIGNIFICANCE: Our data suggest cryptic CHIKV circulation in SJdRP detected by continual active surveillance. These low levels, but increasing, of viral circulation highlight the possibility of CHIKV outbreaks, as there is a large naïve population. Improved knowledge of the epidemiological situation might aid in outbreaks prevention.

ABSTRACT

The objective was to study the composition of sand fly species, the seasonal activity of the dominant species and correlation with average rainfall, sex association of the dominant species with domicile habitats and analyze different landscape scenarios for the two study sites in the municipalities of Itapevi and Mogi of the Crosses. Sandflies were captured with CDC(s) traps installed in three households at each study site for a period of 24 hours, biweekly for two years. A total of 2970 specimens were collected. The dominant species were P.fischeri and M.migonei. A statistically significant difference was registered between males and females of the two species in relation to the intra and peridomicile. The seasonal activity of both species showed a weak significant positive correlation with rainfall. The species P.fischeri and M.migonei may be potential vectors of CVL in the studied sites.

BACKGROUND: Mosquito control is currently the main tool available to contain the spread of several arboviruses in Brazil. We have evaluated the association between entomological surveys of female adult Aedes aegypti and the Breteau index (BI) in space and time in a hyperendemic area, and compared the human resources costs required to measure each of these indicators. METHODS: Entomological surveys were conducted between 2016 and 2019 in Vila Toninho, a neighborhood in the city of São José do Rio Preto, Brazil. Monthly records of collected mosquito specimens were made and then grouped by season. RESULTS: Our findings showed that adult and immature mosquitoes are more related in time than in space, possibly due to differences in their habitats or in climate variables. Bayesian temporal modeling revealed that an increase in 1 standard deviation in the BI was associated with a 27% increase in the number of adult female mosquitoes when adjusted for climatic conditions. The cost of entomological surveys of adult mosquitoes was found to be 83% lower than the cost of determining the BI when covering the same geographic area. CONCLUSIONS: For fine-scale assessments, a simple measure of adult Ae. aegypti abundance may be more realistic than aquatic indicators, but the adult indices are not necessarily the only reliable measure. Surveying adult female mosquitoes has significant potential for optimizing vector control strategies because, unlike the BI, this tool provides an effective indicator for micro-areas within an urban region. It should be noted that the results of the present study may be due to specific features of of the study area, and future studies should analyze whether the patterns found in the study neighborhood are also found in other regions.

ABSTRACT

We conducted mosquito-based Zika virus surveillance in São Paulo State, Brazil from 2015 to 2018. We found 81 pools positive for Aedes aegypti and one pool positive for Aedes albopictus. Infection rates were highest in the summer. Areas with human Zika cases also had clusters of Zika-positive mosquitoes.

ABSTRACT

Mosquito-borne diseases affect millions of individuals worldwide; the area of endemic transmission has been increasing due to several factors linked to globalization, urban sprawl, and climate change. The Aedes aegypti mosquito plays a central role in the dissemination of dengue, Zika, chikungunya, and urban yellow fever. Current preventive measures include mosquito control programs; however, identifying high-risk areas for mosquito infestation over a large geographic region based only on field surveys is labor-intensive and time-consuming. Thus, the objective of this study was to assess the potential of remote satellite images (WorldView) for determining land features associated with Ae. aegypti adult infestations in São José do Rio Preto/SP, Brazil. We used data from 60 adult mosquito traps distributed along four summers; the remote sensing images were classified by land cover types using a supervised classification method. We modeled the number of Ae. aegypti using a Poisson probability distribution with a geostatistical approach. The models were constructed in a Bayesian context using the Integrated nested Laplace Approximations and Stochastic Partial Differential Equation method. We showed that an infestation of Ae. aegypti adult mosquitoes was positively associated with the presence of asbestos roofing and roof slabs. This may be related to several other factors, such as socioeconomic or environmental factors. The usage of asbestos roofing may be more prevalent in socioeconomically poor areas, while roof slabs may retain rainwater and contribute to the generation of temporary mosquito breeding sites. Although preliminary, our results demonstrate the utility of satellite remote sensing in identifying landscape differences in urban environments using a geostatistical approach, and indicated directions for future research. Further analyses including other variables, such as land surface temperature, may reveal more complex relationships between urban mosquito micro-habitats and land cover features.

BACKGROUND: São José do Rio Preto is one of the cities of the state of São Paulo, Brazil, that is hyperendemic for dengue, with the presence of the four dengue serotypes. OBJECTIVES: to calculate dengue seroprevalence in a neighbourhood of São José do Rio Preto and identify if socioeconomic and demographic covariates are associated with dengue seropositivity. METHODS: A cohort study to evaluate dengue seroprevalence and incidence and associated factors on people aged 10 years or older, was assembled in Vila Toninho neighbourhood, São José do Rio Preto. The participant enrolment occurred from October 2015 to March 2016 (the first wave of the cohort study), when blood samples were collected for serological test (ELISA IgG anti-DENV) and questionnaires were administrated on socio-demographic variables. We evaluated the data collected in this first wave using a cross-sectional design. We considered seropositive the participants that were positive in the serological test (seronegative otherwise). We modelled the seroprevalence with a logistic regression in a geostatistical approach. The Bayesian inference was made using integrated nested Laplace approximations (INLA) coupled with the Stochastic Partial Differential Equation method (SPDE). RESULTS: We found 986 seropositive individuals for DENV in 1322 individuals surveyed in the study area in the first wave of the cohort study, corresponding to a seroprevalence of 74.6% (95%CI: 72.2-76.9). Between the population that said never had dengue fever, 68.4% (566/828) were dengue seropositive. Older people, non-white and living in a house (instead of in an apartment), were positively associated with dengue seropositivity. We adjusted for the other socioeconomic and demographic covariates, and accounted for residual spatial dependence between observations, which was found to present up to 800 m. CONCLUSIONS: Only one in four people aged 10 years or older did not have contact with any of the serotypes of dengue virus in Vila Toninho neighbourhood in São José do Rio Preto. Age, race and type of house were associated with the occurrence of the disease. The use of INLA in a geostatistical approach in a Bayesian context allowed us to take into account the spatial dependence between the observations and identify the associated covariates to dengue seroprevalence.

INTRODUCTION: This study aimed to describe the occurrence of human visceral leishmaniasis in Araçatuba with regard to time and space and to identify high risk areas. METHODS: We included all human visceral leishmaniasis autochthonous cases reported between 1999 and 2015. The incidence rates were calculated by sex, age, and year. The human visceral leishmaniasis cases were geocoded and grouped by urban census tracts, enabling the calculation of the incidence and mortality rates by census tracts. For the identification of high risk areas, we utilized the scan statistics and univariate Ripley's K-function. RESULTS: The incidence presented a cyclic pattern in 1999-2009, with peaks in 2002 and 2007 (30.1 and 19.6 cases per 100,000 inhabitant-years, respectively). In 2010-2015, the incidence remained relatively stable with about 2.0 cases per 100,000 inhabitant-years. The scan statistics detected two spatial clusters of high risk and three spatio-temporal clusters of high risk that lasted from 2001 to 2008. A spatial autocorrelation was observed in the human visceral leishmaniasis case point distribution in 1999-2009. No spatio-temporal clusters and no spatial autocorrelation in the case point pattern were identified in 2010-2015. CONCLUSION: We identified a changing pattern of human visceral leishmaniasis occurrence in Araçatuba: the first period (1999-2009) showed a cyclic pattern, clusters, and presence of spatial dependence in the case point distribution; the second period (2010-2015) showed the lowest rates of all historical series, stable incidence, and cases with a random distribution pattern.

ABSTRACT

INTRODUCTION: Traditional indices for measuring dengue fever risk in a given area are based on the immature forms of the vector (larvae and pupae surveys). However, this is inefficient because only adult female mosquitoes actually transmit the virus. Based on these assumptions, our objective was to evaluate the association between an entomological index obtained from adult mosquito traps and the occurrence of dengue in a hyperendemic area. Additionally, we compared its cost to that of the Breteau Index (BI). MATERIAL AND METHODS: We performed this study in São José do Rio Preto, SP, Brazil, between the epidemiological weeks of 36/2012 and 19/2013. BG-Sentinel and BG-Mosquitito traps were installed to capture adult mosquitoes. Positive and negative cases of dengue fever were computed and geocoded. We generated biweekly thematic maps of the entomological index, generated by calculating the number of adult Aedes aegypti females (NAF) per 100 households during a week by kriging, and based on the number of mosquitoes captured. The relation between the occurrence of dengue fever and the NAF was tested using a spatial case-control design and a generalized additive model and was controlled by the coordinates of the positive and negative cases of dengue fever. RESULTS: Our analyses showed that increases in dengue fever cases occurred in parallel with increases in the number of Ae. aegypti females. The entomological index produced in our study correlates positively with the incidence of dengue, particularly during intervals when vector control measures were applied less intensively. The operational costs of our index were lower than those of the BI: NAF used 71.5% less human resources necessary to measure the BI. CONCLUSIONS: Spatial analysis techniques and the number of adult Ae. aegypti females were used to produce an indicator of dengue risk. The index can be applied at various levels of spatial aggregation for an entire study area, as well as for sub-areas, such as city blocks. Even though the index is adequate to predict dengue risk, it should be tested and validated in various scenarios before routine use.

ABSTRACT

Visceral leishmaniasis (VL), a neglected disease, is a serious public health problem that affects millions of people worldwide. The objectives of the study were to evaluate the sensitivity of Lutzomyia longipalpis and canine VL (CVL) autochthony early detection and describe the spatial and temporal dispersal of vector and expansion of VL in a Brazilian state. We obtained data on the leishmaniasis vector and VL cases in São Paulo State (SP), Brazil, from the Division of Endemic Disease Control and from the Epidemiological Surveillance Center of the São Paulo State Department of Health. Data were analyzed for 645 municipalities and 63 microregions and presented as thematic and flow maps. Following the verified presence of L. longipalpis in Araçatuba in 1997, the first autochthonous cases of canine VL (CVL) (1998) and of human VL (HVL) (1999) in São Paulo were reported, both in Araçatuba. From 1997 to 2014, the urban presence of the leishmaniasis vector was verified in 167 (25.9%) municipalities with cases of CVL reported in 108 (16.7%) and cases of HVL in 84 (13%). The sensitivities for vector presence early detection in relation to the identification of CVL and HVL autochthony were, respectively, equal to 76.4 and 92.5%. The sensitivity for CVL autochthony early detection in relation to the HVL autochthony identification was 75.8%. Vector dispersal and expansion of CVL and HVL were from the northwest to the southeast of the state, primarily flanking the Marechal Rondon highway at a constant rate of progression of 10, seven, and six new municipalities affected per year, respectively. We concluded that the sensitivity for vector presence and CVL autochthony presented reasonable accuracy and most of the time the vector presence and, specially, the CVL and HVL autochthony were identified in the main cities of the microregions of SP. Vector dispersal and expansion of VL started in 1997 near the state border of SP with the state of Mato Grosso do Sul. It has advanced from the northwest to the southeast flanking the Marechal Rondon highway at an arithmetic progression rate outward from the main cities of the microregions. Autochthonous cases of CVL and HVL emerged in SP, in general, after the verified presence of L. longipalpis.

OBJECTIVE: To perform both space and space-time evaluations of visceral leishmaniasis in humans in the state of Sao Paulo, Brazil. METHODS: The population considered in the study comprised autochthonous cases of visceral leishmaniasis and deaths resulting from it in Sao Paulo, between 1999 and 2013. The analysis considered the western region of the state as its studied area. Thematic maps were created to show visceral leishmaniasis dissemination in humans in the municipality. Spatial analysis tools Kernel and Kernel ratio were used to respectively obtain the distribution of cases and deaths and the distribution of incidence and mortality. Scan statistics were used in order to identify spatial and space-time clusters of cases and deaths. RESULTS: The visceral leishmaniasis cases in humans, during the studied period, were observed to occur in the western portion of Sao Paulo, and their territorial extension mainly followed the eastbound course of the Marechal Rondon highway. The incidences were characterized as two sequences of concentric ellipses of decreasing intensities. The first and more intense one was found to have its epicenter in the municipality of Castilho (where the Marechal Rondon highway crosses the border of the state of Mato Grosso do Sul) and the second one in Bauru. Mortality was found to have a similar behavior to incidence. The spatial and space-time clusters of cases were observed to coincide with the two areas of highest incidence. Both the space-time clusters identified, even without coinciding in time, were started three years after the human cases were detected and had the same duration, that is, six years. CONCLUSIONS: The expansion of visceral leishmaniasis in Sao Paulo has been taking place in an eastbound direction, focusing on the role of highways, especially Marechal Rondon, in this process. The space-time analysis detected the disease occurred in cycles, in different spaces and time periods. These meetings, if considered, may contribute to the adoption of actions that aim to prevent the disease from spreading throughout the whole territory of São Paulo or to at least reducing its expansion speed. OBJETIVO: Avaliar, no espaço e espaço-tempo, a ocorrência da leishmaniose visceral em humanos no estado de São Paulo, Brasil. MÉTODOS: Considerou-se como população de estudo os casos autóctones e óbitos por leishmaniose visceral em humanos ocorridos em São Paulo entre 1999 e 2013 e, como área de estudo, a região oeste do estado. Construíram-se mapas temáticos que mostraram a disseminação da leishmaniose visceral em humanos nos municípios. Ferramentas de análise espacial Kernel e razão Kernel foram utilizadas para obter, respectivamente, a distribuição dos casos e óbitos e a distribuição da incidência e mortalidade. Utilizaram-se estatísticas de varredura para identificar aglomerados espaciais e espaço-temporais de casos e óbitos. RESULTADOS: Os casos de leishmaniose visceral em humanos, no período de estudo, ocorreram na parte ocidental de São Paulo e sua expansão territorial seguiu principalmente o curso da rodovia Marechal Rondon, no sentido oeste-leste. As incidências foram caracterizadas como duas sequências de elipses concêntricas com intensidade decrescente. A primeira, com maior intensidade, teve epicentro no município de Castilho (cruzamento da rodovia Marechal Rondon com a divisa com o estado de Mato Grosso do Sul) e a segunda, em Bauru. A mortalidade apresentou comportamento similar ao da incidência. Os aglomerados espaciais e espaço-temporais de casos coincidiram com as duas áreas de maiores incidências. Ambos os aglomerados espaço-temporais identificados, mesmo sem coincidirem temporalmente, tiveram início após três anos da detecção dos casos humanos e tiveram a mesma duração, seis anos. CONCLUSÕES: A expansão da leishmaniose visceral em São Paulo vem ocorrendo no sentido oeste-leste, com destaque ao papel das rodovias, especialmente a Marechal Rondon, neste processo. A análise espaço-temporal detectou, em diferentes espaços e períodos de tempo, ciclicidade na ocorrência da doença. Esses encontros, se considerados, podem colaborar para a adoção de ações que visem a evitar que a doença se espalhe por todo o território paulista ou, no mínimo, diminuir sua velocidade de expansão.

INTRODUCTION This paper aims to describe the dispersion of Lutzomyia longipalpis and the autochthonous occurrence of visceral leishmaniasis (VL) in the Northwest region of the State of São Paulo between 2007 and 2013 and to analyze their expansion. METHODS Information about the vector and associated cases was described using maps. The incidence, mortality, and lethality of human visceral leishmaniasis (HVL) were calculated. In municipalities in which more than one HVL case occurred, incidences were calculated according to census sector, and spatial and spatiotemporal clusters were identified. RESULTS The first case of HVL was reported in the municipality of Jales in 2007. By 2013, the vector and the disease had expanded from west to east, with the vector being detected in 29 municipalities. A total of 11 municipalities had cases of canine visceral leishmaniasis (CVL), and six had cases of HVL. Vector expansion occurred by vicinity with previously infested municipalities, and the expansion of VL was related to the major highways and the capital municipalities of the micro-regions in the study area. The highest incidence of HVL occurred in children between 0-4 years old, and the highest mortality and lethality occurred among persons aged 60 and older. The occurrence of HLV was more intense in the peripheral areas of municipalities with the disease. CONCLUSIONS The findings of this study may be useful for improving VL surveillance and control activities by slowing VL expansion and/or mitigating VL effects when they occur.

Abstract: INTRODUCTION This paper aims to describe the dispersion of Lutzomyia longipalpis and the autochthonous occurrence of visceral leishmaniasis (VL) in the Northwest region of the State of São Paulo between 2007 and 2013 and to analyze their expansion. METHODS Information about the vector and associated cases was described using maps. The incidence, mortality, and lethality of human visceral leishmaniasis (HVL) were calculated. In municipalities in which more than one HVL case occurred, incidences were calculated according to census sector, and spatial and spatiotemporal clusters were identified. RESULTS The first case of HVL was reported in the municipality of Jales in 2007. By 2013, the vector and the disease had expanded from west to east, with the vector being detected in 29 municipalities. A total of 11 municipalities had cases of canine visceral leishmaniasis (CVL), and six had cases of HVL. Vector expansion occurred by vicinity with previously infested municipalities, and the expansion of VL was related to the major highways and the capital municipalities of the micro-regions in the study area. The highest incidence of HVL occurred in children between 0-4 years old, and the highest mortality and lethality occurred among persons aged 60 and older. The occurrence of HLV was more intense in the peripheral areas of municipalities with the disease. ABSTRACT OBJECTIVE To perform both space and space-time evaluations of visceral leishmaniasis in humans in the state of Sao Paulo, Brazil. METHODS The population considered in the study comprised autochthonous cases of visceral leishmaniasis and deaths resulting from it in Sao Paulo, between 1999 and 2013. The analysis considered the western region of the state as its studied area. Thematic maps were created to show visceral leishmaniasis dissemination in humans in the municipality. Spatial analysis tools Kernel and Kernel ratio were used to respectively obtain the distribution of cases and deaths and the distribution of incidence and mortality. Scan statistics were used in order to identify spatial and space-time clusters of cases and deaths. RESULTS The visceral leishmaniasis cases in humans, during the studied period, were observed to occur in the western portion of Sao Paulo, and their territorial extension mainly followed the eastbound course of the Marechal Rondon highway. The incidences were characterized as two sequences of concentric ellipses of decreasing intensities. The first and more intense one was found to have its epicenter in the municipality of Castilho (where the Marechal Rondon highway crosses the border of the state of Mato Grosso do Sul) and the second one in Bauru. Mortality was found to have a similar behavior to incidence. The spatial and space-time clusters of cases were observed to coincide with the two areas of highest incidence. Both the space-time clusters identified, even without coinciding in time, were started three years after the human cases were detected and had the same duration, that is, six years. CONCLUSIONS The expansion of visceral leishmaniasis in Sao Paulo has been taking place in an eastbound direction, focusing on the role of highways, especially Marechal Rondon, in this process. The space-time analysis detected the disease occurred in cycles, in different spaces and time periods. These meetings, if considered, may contribute to the adoption of actions that aim to prevent the disease from spreading throughout the whole territory of São Paulo or to at least reducing its expansion speed.


RESUMO OBJETIVO Avaliar, no espaço e espaço-tempo, a ocorrência da leishmaniose visceral em humanos no estado de São Paulo, Brasil. MÉTODOS Considerou-se como população de estudo os casos autóctones e óbitos por leishmaniose visceral em humanos ocorridos em São Paulo entre 1999 e 2013 e, como área de estudo, a região oeste do estado. Construíram-se mapas temáticos que mostraram a disseminação da leishmaniose visceral em humanos nos municípios. Ferramentas de análise espacial Kernel e razão Kernel foram utilizadas para obter, respectivamente, a distribuição dos casos e óbitos e a distribuição da incidência e mortalidade. Utilizaram-se estatísticas de varredura para identificar aglomerados espaciais e espaço-temporais de casos e óbitos. RESULTADOS Os casos de leishmaniose visceral em humanos, no período de estudo, ocorreram na parte ocidental de São Paulo e sua expansão territorial seguiu principalmente o curso da rodovia Marechal Rondon, no sentido oeste-leste. As incidências foram caracterizadas como duas sequências de elipses concêntricas com intensidade decrescente. A primeira, com maior intensidade, teve epicentro no município de Castilho (cruzamento da rodovia Marechal Rondon com a divisa com o estado de Mato Grosso do Sul) e a segunda, em Bauru. A mortalidade apresentou comportamento similar ao da incidência. Os aglomerados espaciais e espaço-temporais de casos coincidiram com as duas áreas de maiores incidências. Ambos os aglomerados espaço-temporais identificados, mesmo sem coincidirem temporalmente, tiveram início após três anos da detecção dos casos humanos e tiveram a mesma duração, seis anos. CONCLUSÕES A expansão da leishmaniose visceral em São Paulo vem ocorrendo no sentido oeste-leste, com destaque ao papel das rodovias, especialmente a Marechal Rondon, neste processo. A análise espaço-temporal detectou, em diferentes espaços e períodos de tempo, ciclicidade na ocorrência da doença. Esses encontros, se considerados, podem colaborar para a adoção de ações que visem a evitar que a doença se espalhe por todo o território paulista ou, no mínimo, diminuir sua velocidade de expansão.

ABSTRACT

The aims of this study were to describe the occurrence of dengue in space and time and to assess the relationships between dengue incidence and entomologic indicators. We selected the dengue autochthonous cases that occurred between September 2005 and August 2007 in São José do Rio Preto to calculate incidence rates by month, year and census tracts. The monthly incidence rates of the city were compared to the monthly Breteau indices (BI) of the São José do Rio Region. Between December 2006 and February 2007, an entomological survey was conducted to collect immature forms of Aedes aegypti in Jaguaré, a São José do Rio Preto neighborhood, and to obtain entomological indices. These indices were represented using statistical interpolation. To represent the occurrence of dengue in the Jaguaré neighborhood in 2006 and 2007, we used the Kernel ratio and to evaluate the relationship between dengue and the entomological indices, we used a generalized additive model in a spatial case-control design. Between September 2005 and August 2007, the occurrence of dengue in São José do Rio Preto was almost entirely caused by DENV3, and the monthly incidence rates presented high correlation coefficients with the monthly BI. In Jaguaré neighborhood, the entomological indices calculated by hectare were better predictors of the spatial distribution of dengue than the indices calculated by properties, but the pupae quantification did not show better prediction qualities than the indices based on the container positivity, in relation to the risk of dengue occurrence. The fact that the municipality's population had a high susceptibility to the serotype DENV3 before the development of this research, along with the almost total predominance of the occurrence of this serotype between 2005 and 2007, facilitated the analysis of the epidemiological situation of the disease and allowed us to connect it to the entomological indicators.

Realizou-se a análise de conteúdo desses registros, identificando-se cinco categorias: inserção social; integralidade da atenção; intersetorialidade; valorização dos AS; e educação permanente. Os profissionais expressaram a necessidade dos fóruns de discussão serem permanentes; as dificuldades para realizar ações intersetoriais; e o sentimento de valorização profissional por participar das equipes das UBS e por colaborar com a participação comunitária. This study analyzes the insertion process of vector control agents into the primary health centers (PHCs) in São José do Rio Preto, São Paulo, Brazil, designated health workers (HWs), in order to improve the effectiveness of the dengue fever control program. This is a case study, based on direct observation and recording of participants’ speeches in forums held by the municipal Secretary of Health with these professionals. These records underwent content analysis, and five categories were identified: social insertion; comprehensiveness of care; intersectoriality; appreciation of HW; and continuing education. The professionals expressed the need for permanent discussion forums; the difficulties to take intersectoral actions; and the feeling of professional appreciation for participating in teams from PHCs and collaborating to community participation. The study allowed us to understand how the insertion of HWs took place in municipal primary care and the complexity of dengue fever control, at this level of care, which involves socio-environmental issues and intersectoral action. INTRODUCTION: In 1999, Birigui and Araçatuba were the first municipalities in the State of São Paulo to present autochthonous cases of visceral leishmaniasis in humans (VLH). The aim of this study was to describe the temporal, spatial and spatiotemporal behaviors of VLH in Birigui. METHODS: Secondary data were obtained from the Notifiable Diseases Information System from 1999 to 2012. The incidence, mortality and case fatality rates by sex and age were calculated. The cases of VLH were geocoded and grouped according to census tracts. Local empirical Bayesian incidence rates were calculated. The existence of spatial and spatiotemporal clusters was investigated using SaTScan software. RESULTS: There were 156 confirmed cases of autochthonous VLH. The incidence rate was higher in the 0-4-year-old children, and the mortality and case fatality rates were higher in people aged 60 years and older. The peaks of incidence occurred in 2006 and 2011. The Bayesian rates identified the presence of VLH in all of the census tracts in the municipality; however, spatial and spatiotemporal clusters were found in the central area of the municipality. CONCLUSIONS: Birigui, located in the Araçatuba region, has recently experienced increasing numbers of VLH cases; this increase is contrary to the behavior observed over the entire region, which has shown a decreasing trend in the number of VLH cases. The observations that the highest incidence is in children 0-4 years old and the highest mortality is in people 60 years and older are in agreement with the expected patterns of VLH.
